Guardian.

Life Insurance
|
Radwell
provides employees a $20,000 group term Life/AD&D insurance policy at no
cost to the employee.

|
For
employee, spouse and dependent children paid 100% by employee.

FSA (Flexible Spending Account)
|
Allows
eligible employees to set aside a specific pretax dollar amount
for unreimbursed medical, dental, vision, over the counter items, and
dependent care expenses.

Accruing and Using PTO:
PTO is accrued upon date of
hire. Eligible employees must be
scheduled to work at least forty (40) hours per week on a regular basis. PTO may be used in day or partial day
increments.
PTO
accruals are available for use in the pay period following completion of 90
days of employment. All hours
thereafter are available for use in the pay period following the pay period
in which they are accrued. Employees
accumulate a specified amount of PTO each pay period worked. PTO is accrued on a pro-rated basis
throughout the year.
